The short answer: a double chin is a description, not a diagnosis
If the fullness beneath the chin is mainly localized fat, a physician may discuss a contouring approach differently from true skin or neck-tissue laxity. If both contribute, neither device name alone explains the plan. Bone structure, muscle position, skin quality, previous weight change and prior procedures can also change the appearance.
ONDA and Ultherapy Prime therefore should not be ranked as if one were simply the stronger version of the other. ONDA uses microwave energy and manufacturer materials describe specific handpieces for submental fat and tightening. Ultherapy Prime uses focused ultrasound with real-time imaging and has an FDA-cleared indication for lifting lax submental and neck tissue.
A friendly way to begin is: ‘What tissue are we trying to change, and how did you decide?’ A useful consultation can answer that question, explain what neither device can correct and include waiting or no treatment as reasonable options.
ONDA: why the handpiece and treatment target matter
DEKA describes ONDA as a 2.45 GHz microwave platform. Its current product information separates Deep and Shallow handpieces, with the Shallow handpiece listed for skin tightening and submental treatment. ONDA PRO adds a Pocket handpiece described for more precise firming and submental work. Those names matter because ‘ONDA on the face’ is not a complete treatment record.
A 2022 study followed 48 people receiving six submental microwave sessions two weeks apart, with assessment 12 weeks after the last session. The investigators reported lower fat and laxity scores and 33 of 47 completers were satisfied or very satisfied. It was not a direct comparison with Ultherapy, and a multi-session study cannot prove what one session will do for an individual.
A separate 2025 report included only ten women who received two sessions 30 days apart. It adds small, early evidence for submental laxity, not a universal protocol. Ask the clinic to name the ONDA model, handpiece, treatment area, planned sessions and reason for choosing it.

Ultherapy Prime: imaging depth is not the same as treatment depth
Ultherapy Prime combines focused ultrasound therapy with real-time ultrasound visualization. The current instructions for use state that imaging can visualize tissue to 8 mm, while treatment transducers place energy at selected treatment depths. An 8 mm image does not mean the device treats every person at 8 mm.
The US FDA-cleared indications include lifting lax tissue beneath the chin and on the neck. That wording is important: it addresses lax tissue, not a promise to remove every form of submental fullness. A thick fat layer, prominent gland, muscle anatomy or skeletal profile can call for a different discussion.
Ask which transducer depths and regions are planned, what the live image shows and why the physician selected the line pattern. A large line count cannot substitute for appropriate anatomy, contact and technique.

What the evidence can say—and what it cannot compare
We did not find a peer-reviewed ONDA-versus-Ultherapy Prime head-to-head trial in the sources reviewed on 17 September 2026. Separate studies cannot be converted into a winner because the participants, treatment targets, handpieces, transducers, session counts and outcome scales differ.
The 2024 ONDA advisory-board review says that long-term efficacy and safety evaluation was still missing and presents recommendations from experienced users. That is useful for understanding selection and technique, but it is not independent comparative proof. The small submental studies also need confirmation in larger controlled trials.
Ultherapy has a separate device record, labeling and evidence base. FDA clearance establishes the reviewed indications and device pathway; it does not guarantee an individual cosmetic result or prove superiority over a microwave system. Marketing words such as ‘fat melting,’ ‘deep lift’ or ‘no downtime’ should be translated into a named tissue target, device setting and realistic range of outcomes.
Safety: non-invasive does not mean risk-free
For ONDA, the 2024 expert review lists medical and local skin conditions that may lead a physician to postpone or rule out treatment, including implanted electronic devices, certain serious illnesses and active skin problems in the treatment area. It also notes that localized heat and discomfort must be monitored. The full device instructions and local labeling should guide the treating clinician.
Ultherapy's current instructions list common clinical-trial reactions such as redness, swelling, pain and transient nerve effects. Post-market reports also include burns or blisters, nodules, fat or volume loss, pigment change, scarring, numbness, weakness and other neurological symptoms; reported severity and duration vary, and voluntary reports cannot establish how often an event occurs.
Tell the physician about implants, pregnancy, active infection or inflammation, altered sensation, relevant medicines and every recent filler, thread, surgery, ultrasound, RF or fat-reduction treatment by date and area. Seek prompt medical assessment for severe or worsening pain, a burn or blister, unusual color change, spreading redness, weakness, numbness, trouble speaking, vision change or another rapidly worsening symptom.
Autumn travel planning: leave room for review, not just a result
September and October can be easier months for walking in Seoul than midsummer, but cooler weather does not remove treatment risk. Do not place a first facial energy treatment immediately before a long-haul flight, wedding, close-up photography or a tightly scheduled departure.
Share arrival, event and flight dates before booking. Ask for a realistic recovery range, written aftercare in English and an in-person review window. If the itinerary cannot hold that margin, consultation only may be the more considerate plan.

Before you book
Is ONDA or Ultherapy better for a double chin?
Neither is universally better. ONDA may be discussed when localized submental fat and tightening are relevant, while Ultherapy Prime is indicated for lifting lax tissue beneath the chin and on the neck. Anatomy, tissue cause, history and device plan require physician assessment.
Does Ultherapy remove double-chin fat?
Its US indication is to lift lax submental and neck tissue, not to promise removal of every form of fullness. A physician should distinguish fat, lax tissue, muscle, gland and skeletal structure before recommending treatment.
How many ONDA sessions are needed under the chin?
There is no universal schedule. Published submental studies used six sessions two weeks apart or two sessions 30 days apart, but those small studies do not prescribe one best plan. Confirm the model, handpiece, area and evidence for the proposed schedule.
Does Ultherapy Prime treat at 8 mm?
The current instructions say the system can visualize tissue to 8 mm. Visualization depth and treatment depth are different. Ask which transducer and treatment depth are planned for each region and why.
Can ONDA and Ultherapy be done together?
A combination is not automatically better. If fat and lax tissue both contribute, a physician may discuss staging, but the sequence and interval depend on anatomy, settings, recent procedures, recovery and local labeling. Doing more in one trip can also complicate follow-up.
What should be written in the quote?
Ask for the exact device model, ONDA handpiece or Ultherapy transducer, treatment area, session or line count, clinician, anesthesia, VAT, aftercare, review and urgent-contact route. A device name and total price are not enough for a fair comparison.
